Melbourne City Mission’s experienced Support Coordinators can work with you to get your National Disability Insurance Scheme (NDIS) plan up and running.
This service ensures you can access the supports you need and live the life you choose.
We've been there from the beginning of the NDIS and are here to help you get the most from your NDIS Plan.
Our knowledgeable and reliable NDIS Support Coordinators are committed to working with and for you to achieve positive outcomes. Your Support Coordinator will take the time to get to know you, your goals, interests and needs to find the right solutions for you.
We also have experienced Specialist Support Coordinators available to assist you. Specialist Support Coordination is a time limited service designed for people who have more intensive or complex needs.
Our specialists use their expertise to support you to address barriers, overcome challenges and reduce risks and complexity associated with your personal circumstances. This may mean connecting you with a range of supports — including health, education or justice services — to ensure everything works well together.
When you start with MCM, we’ll spend some time with you face-to-face to get to know you. We’ll talk about your goals and needs, and discuss the services you would like to access.
Your Support Coordinator will check in with you and your service providers regularly to make sure things are working well. Reports are produced during key milestones, and a meeting is organised near the end of your plan to talk about your future goals and needs.
Support Coordination is funded by the National Disability Insurance Agency (NDIA) and is allocated to assist you with implementing your plan. When you’re developing your NDIS Plan, talk to your planner about the assistance you may need.
Do you have other sources of funding, such as Transport Accident Commission (TAC), WorkCover or Department of Veterans Affairs funding? We can assist you to coordinate your services as needed.
