Make a referral

Anyone can make a palliative care referral, providing the person receiving the care agrees.

The person receiving care must be:

• diagnosed with a life limiting illness
• living within the following local government areas — Hume, Merri-bek, Darebin or Yarra

To make a referral

Online portal

On the online referral portal, you will need to provide personal and medical information about the client.

Please complete as many fields as possible as this will assist us in assessing your needs accurately.

Mandatory fields must be completed.

Make a referral

Please upload copies of all relevant medical information, such as:

• a current medication list
• hospital admission sheet
• copies of scan results
• recent pathology results
• relevant specialist letters and doctor’s notes.

This information is needed for clinician referrals and will ensure that your submission is processed quickly.

Melbourne City Mission’s Palliative Care services are provided at no cost to clients who live in the Hume, Merri-bek, Darebin and Yarra local government areas.
