Melbourne City Mission Palliative Care provides a 24 hour service 7 days per week. Staff will be required to working morning shifts  and  will include evening shifts once implemented into the service mid to late 2023. 

 

Reporting to the Coordinator Nursing Services as a Community Palliative Nurse  this role is responsible to Working as a member of an interdisciplinary team and in collaboration with external health professionals, the Palliative Care RN will provide palliative care to MCMPC clients in accordance with the mission, values, policies, and procedures of MCM Palliative Care and within the philosophy and organisational values of Melbourne City Mission across Victoria.
Duties include but are not limited to:

  • Provide comprehensive assessment and regular review of palliative care clients - using a holistic and anticipatory approach
  •   Plan and coordinate services (with other agencies or disciplines) relating to end of life care with all clients and carers
  • Deliver a high standard of care regardless of clients’ or colleagues’ differing values, beliefs, culture and social context
  •   Ability to work independently with guidance and support from the senior nursing team.

About you

We are looking for someone who has:

  • Nursing experience in a community setting
  • Demonstrated knowledge of aged care and/or chronic illness

Essential:

  • Minimum 3 years postgraduate work experience in palliative care or oncology.
  • Strongly desirable but not essential experience in community nursing.
  • Demonstrated high level of skill in clinical decision-making and assessment skills.
  • Excellent communication skills.
  • Ability to work collaboratively within an interdisciplinary team setting.
  • Sensitivity to people living with a terminal condition and those who are dying or bereaved.

  • National Police check
  • Working with Children check
  • Valid Victorian Drivers license
  • Right to Work in Australia
  • Up to date with COVID-19 Vaccinations or valid medical exemption 
  • MCM employees are required to be fully vaccinated against COVID-19.
