This position is located within the Melbourne City Mission Disability, Early Years and Palliative Care Division and implements Melbourne City Mission’s commitment to best practice in palliative care.

Melbourne City Mission Palliative Care (MCMPC) was established in 1981 as the first community-based palliative care service to operate in Victoria. MCMPC aims to enhance quality of life by providing specialist interdisciplinary palliative care for people living with a terminal condition, their families and carers.

MCMPC provides a 24 hour service 7 days a week.

About the role

An exciting opportunity to lead an innovative project aimed at providing an alternative access point to community palliative care services using Telehealth. With a focus on interaction with clients with long term chronic diseases that require sporadic connection to community palliative care this role will aim to implement the PalCare Go telehealth and PalCare at Home capability to improve access to specialist palliative care for this cohort of clients.

  • The identification of clients with long term chronic illnesses that require a palliative care response during a longer trajectory
  • Review of current practice and development of procedures to implement the use of Telehealth as an alternative for clients with chronic diseases
  • Work with the PalCare Go Project Managers in reviewing data, outcomes and embedding practice
  • Develop key stakeholder relationships to build relationships, generate and expand client base
  • Develop training resources to assist staff and client orientation to the service

Skills and experience

Essential:

  • Experienced Registered Nurse in Palliative Care, with preference to experience in:
    • Health Independence Programs (HIP), such as HARP (Hospital Admission Risk Program) who work closely with chronic non-malignant disease populations related to Respiratory, Cardiac and Renal disease 
    • Community nursing 
  • Strong client focus with the ability to deliver client-lead service responses 
  • Strong problem-solving skills,  particularly  in the implementation of a new technology driven service or program
  • Experience in the development of procedure, protocols and key communication tools

Desirable:

  • Project Management experience
