This position is located within the Melbourne City Mission Disability, Early Years and Palliative Care Division and implements Melbourne City Mission’s commitment to best practice in palliative care.
Melbourne City Mission Palliative Care (MCMPC) was established in 1981 as the first community-based palliative care service to operate in Victoria. MCMPC aims to enhance quality of life by providing specialist interdisciplinary palliative care for people living with a terminal condition, their families and carers.
MCMPC provides a 24 hour service 7 days a week.
An exciting opportunity to lead an innovative project aimed at providing an alternative access point to community palliative care services using Telehealth. With a focus on interaction with clients with long term chronic diseases that require sporadic connection to community palliative care this role will aim to implement the PalCare Go telehealth and PalCare at Home capability to improve access to specialist palliative care for this cohort of clients.
